Eligibiligty Rules

U14 Tournament • Match Format: 5v5 including goalkeeper • Maximum 12 players may be registered • Maximum 2 U15 players may be registered • Only ONE (1) U15 player may be on the field at any time • Date of Birth required for all players • Proof of age may be requested

N.B. ALL ENTRY FORMS MUST BE ACCOMPANIED BY BIRTH CERTIFICATES/PASSPORTS, PHOTOGRAPHS OF PARTICIPANTS & ENTRY FEE

I confirm only one U15 on field at a time

I, the undersigned, confirm that I am the authorized representative (Coach/Team Manager) registering players for participation in Youth Football League tournaments, including but not limited to the Girlz Can Play Too Tournament, Next Cup, and Champions Cup.

I confirm that:

• I have obtained full consent and approval from each player’s parent and/or legal guardian for the player’s participation.
• All players registered are medically fit and able to participate in football activities.
• I have the authority to act on behalf of all registered players and their parents/guardians in matters relating to this tournament.

I understand and agree that participation in football and related activities involves inherent risks, including but not limited to injury, illness, or accident.

On behalf of myself, the registered players, and their parents/guardians, I hereby:

• Assume all risks associated with participation in the tournament.
• Release, waive, and discharge the Youth Football League, FYI Consultancy Group, event organizers, venue owners, officials, volunteers, partners, and sponsors from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained during participation.

I further agree to abide by all tournament rules, regulations, and decisions made by the organizers.

By completing registration, I acknowledge that I have read, understood, and agree to this Release of Liability and Consent in full.
